Identification Requirements:
A Driver’s License or State Identification or U.S. Passport
ALSO:
A Birth Certificate or Naturalization Papers
All certificates and papers must be originals. The Department of State will not accept copies.
Birth Certificates must have a raised seal AND a file date.
These documents must be submitted with the application and will be returned.
