We will be communicating with you via E-mail in the coming weeks concerning flight information, course information and more. But first, to participate in any study abroad program, you must complete the forms detailed below and return them to our offices: Academic Programs Abroad, 103 Boone Hall, Ypsilanti, Michigan 48197.

The following is a checklist of items that must be returned to us. You will find more information on these items below:

  • Agreement and Release Form-Fill out and return to our office
  • Handbook Agreement Form-Read the online handbook and then fill out the agreement form and return it to our office
  • International Student ID (ISIC) form Please fill it out and return it to our office
  • Request for 2 photos-Return requested photos to our office
  • Health Form Please fill out and return to our offices.

The Forms:

AGREEMENT AND RELEASE FORM All students must print and fill out the AGREEMENT AND RELEASE FORM. Once you and/or your parents have filled it out please return it to our office.

ONLINE HANDBOOK AND HANDBOOK AGREEMENT FORM All students must read the ONLINE HANDBOOK. This handbook is designed to answer questions you may have about the program and to outline your rights and responsibilities. It includes a packing list, the student conduct code and other helpful information. You may want to print out a copy to take with you on the program. Once you have read the handbook, print out the HANDBOOK AGREEMENT FORM, fill it out and return it to our office.

INTERNATIONAL STUDENT IDENTITY CARD (ISIC) The International Student Identity Card (ISIC) is essential to the program because of the discounts that it allows us to sites and museums all around the world. You must print and complete the pdf document linked above and return it to our office as soon as possible. The cards will be kept by the tour manager, who will then distribute the cards to the students at the conclusion of the tour. It is possible to get discounted flights abroad through student agencies using your ISIC number. For any students using such agencies, you will need to provide your ISIC number to purchase tickets. Please phone our office for this number, if needed, or if you have further questions regarding this card. We can not distribute this number until we have received your completed application. REQUEST FOR TWO PHOTOS

We need participants to send our office two photographs (about 1 1/2" x 2"). These photos will be used for various IDs and transportation passes. Your photos must be full frontal head shots. Black and white or colored photographs will be accepted. Passport photos will work fine, but they tend to be expensive. For our purposes photo booth pictures or your own photos will be adequate and much cheaper. Please print and sign your name on the back of each photo. Also, it is never a bad idea to have a couple of extra photos with you while you are traveling in case of emergency. See sample photo:

PAYMENT INFORMATION

When accepted into the Study Abroad Program, separate charges will appear on your student account for the Study Abroad Program Fee and EMU tuition and fees. Billing addresses are no longer used for student account bills. Students will receive an online bill via their MyEmich account. If you are not accepted into the program to which you are applying, or if that program is cancelled, you are not responsible for program charges. Payments are due on billing and must be made directly to the Cashier's Office (payable to Eastern Michigan University) 201 Pierce Hall, Eastern Michigan University, Ypsilanti, Michigan 48197, or may be paid online.

Billing cycles are determined by EMU Student Business Services in accordance with a quarter system of Fall, Winter, Spring and Summer semesters, which may not correspond directly with the dates of your Study Abroad Program. Late payment will generate late fees. Any questions regarding billing or viewing your account should be referred to EMU Student Business Services: Telephone: 734.487.3335.

In order to view your online bill (Ebill) you will have to set up your my.emich.edu account and email.

my.emich - EID and PIN Retrieval
Your EID
Your student EID is a unique identifier assigned to you. Newly admitted or re-enrolling students receive their student EID in their admission letter. The EID has 9 characters. The first character is an "E".
Your PIN
Your PIN is a Personal Identification Number assigned only to you. The PIN number is 6 digits long. The PIN is used as the password to some systems on campus.
Uses of your EID and PIN
The EID and PIN are used to access some features on the campus website. You use your EID and PIN to establish your my.emich account, reset your my.emich account password should you forget it, and to access your online bill through ebill.emich.edu.

How do I get my EID and PIN? You should have received your EID and PIN in separate letters from Admissions. If you do not have your EID and PIN, you can contact the Records and Registration office at 734-487-2300.


If you already have a my.emich username and password, you can look up your EID and PIN. Follow the directions below:
1. Log in to my.emich
2. Select the Student Tab
3. Go to the Service Main Menu
4. Select Personal Information
5. Select Display EID and PIN

PASSPORTS

In the event you have not yet applied for your passport, please do so immediately. Applications can be obtained at any county courthouse, passport agency or designated post office. For complete passport information, including printable application forms, see the State Department Passport web site at: http://travel.state.gov/passport/passport_1738.html

If you currently have a passport, please check and make sure that it is valid for six months after the end of the program. Please remember to sign your passport.


Flights

You must complete all necessary final paperwork(see above) to participate in a study abroad program. In the coming weeks, we will E-mail you more course information, flight information and a participant list. Please do not purchase flights before recieving flight information from our office.
