In order to receive accommodation you must provide documentation demonstrating that you have an impairment or condition that substantially limits you in one or more major life activities.

Documentation

In order to establish that you have a disability and qualify for accommodation your documentation MUST:

Be from a health-care professional who is licensed/certified to treat the underlying condition or impairment for which you seek accommodation

Contain

A diagnosis of the condition or impairment for which you seek accommodation and

A description of the functional limitations associated with that diagnosis

Be on your health-care professionals letterhead

Bear an original signature

Be no more than five years old
