		The Admissions Forms Library
		Forms Library contains all the forms you need to apply for admission or change your admission status. To view and print the forms, you can choose to use Adobe Acrobat Reader or print the online version. If you don't already have the reader, click the Acrobat Reader icon, then follow the instructions from Adobe to download and install the software. After you've finished the install, return to this page to obtain your forms. When a form is displayed in your browser, use the Save As function to save it to your computer. You can then open the form while offline, type the information into it, print and mail. If you cannot save the form, use the Print function of your browser to print a copy. Fill in the form, then mail.

		Forms that can be used by Graduate or Undergraduate International Students

		Statement of Civil/Criminal Issues (Formerly known as the Moral Turpitude Statement) Required for students who are applying to the College of Education. Disciplinary Form Eastern Michigan University's admission application includes questions regarding prior criminal convictions and /or disciplinary history. The information provided by the student may be considered when making the decision as to whether to admit the student to the University. This form is required for all applications starting with the Fall 2001 semester. If you completed an old application or neglected to complete the questions you may complete this form and FAX it to the univeristy at FAX number: 734-487-6559. Financial Form for International Applicants Required for international students seeking or trying to maintain an F-1 or J-1 student visa. Foreign Student Advisor Letter The Foreign Student Advisor Letter is required of all international students who are currently studying in the United States and planning to be an F-1 or J-1 student at EMU. This form should be completed by the foreign student advisor at the school you are currently attending. English as a Second Language Factsheet (ESL) Information for international applicants about the English as a Second Language Program Release Form The International Release Form can be used by International Students to give permission for the university to release information about the applicants application status to another person named on the International Release Form. Transcript Request Form The Transcript Request Form can be used by applicants to request their official academic record from the institutions they previously attended.

		Admission Checklist for Undergraduate International Students A checklist for international undergraduate students Undergraduate Application for Admission Freshmen, transfer, second bachelor's, Post-Bachelors Teacher Certification, and international students who are pursuing a course of study at the bacalaureate level or below. Undergraduate Application Update Form For students who have already submitted an application but need to change the semester they plan to attend. Undergraduate Guest Application For students who are currently enrolled in a degree program at another instution and wish to enroll at EMU for one semester. Reenrollment Form For admitted undergraduate students, including those in a second bachelor or post-degree certification program, who have attended EMU previously, but not enrolled during the last two years (eight semesters).	Admission Checklist for Graduat e International Students A checklist for international undergraduate students Graduate Application for Admission Application for all students who have already completed a bachelor's degree and who wish to continue their studies at a higher level. Graduate Student Enrollment Change Form For graduate students who need to change the semester they plan to attend, re-enroll, or change their progam of concentration. Masters in Social Work Recommendation Form Required for admission in to the MSW program. See the MSW dept site for additional requirements. Masters in Social Work Record of Employment and volunteerism Required for admission in to the MSW program. See the MSW dept site for additional requirements. Post-Baccalaureate Teacher Certification The department application which is required for entry into the post baccaalaureate teacher certification program. This is in addition to the university application which is required for admission to the university.
