Course
New/Change Forms
These
forms have been ported from the EMU Academic
Affairs website for use by the faculty
in the College of Education. The forms
appear in two formats. The Adobe Reader
.pdf format and the Microsoft Word .doc
format. The forms are identical with
the ones found at the Academic Affairs
site except that you can fill in the
forms using your computer.

Fillable Format
These
forms require:

Adobe
Reader, available
at http://www.adobe.com/reader
Click
on the Adobe Reader button to download
the reader. Once the reader has been
installed you can open these files.
- Internet
Explorer 5 or above, Netscape
7 or above, Firefox for either Mac or
Windows, or Safari, for Macintosh.
You
may type on the form while you are online,
or you may download the pdf file to your
desktop to be filled in off-line. You
must, however, complete the form in one
session or print out the whole form and
complete using a typewriter.
There
is no provision for saving these
forms with the free Adobe Reader
program.
Move
through the fields on the form by using
the tab key to advance or the shift +
tab key to go back. You may also use
your mouse to move through the fields.
At
present, there is no provision for submitting
these forms electronically. You must
print out the finished form and submit
in the normal fashion.
Online
Fillable Forms in Microsoft
Word (.doc) Format
These
forms can be used by Microsoft Word, on either Mac or Windows machines.
They are in .doc format and
you are able to fill in the forms using the word processing application you have on your office
machine.
To
use these forms you download the form
by clicking on the .doc link. The form
will be downloaded to your machine and
you can then fill in, save, and/or print
out the form.
The
advantage to using this format is that
you can work on the form over a period
of time, saving your work as you go.
It is advisable to download the original
form, then save the form under
another name using the "Save As..." command.
Fill
in the form by clicking on the gray areas
and typing in your information (be sure
that the gray area turns black before
typing.) Don't worry about the size of
the gray area, it will expand to hold
your response.
