State Periodic Review/Program Evaluation Forms

These forms are in .rtf (Word and Wordperfect) format. To download the files to your desktop for filling in, click on the links below. A dialog box asking whether to open or save the form will open. Choose the save command. Make note of where the form is downloading to your desktop. Then open the form in either Microsoft Word or Wordperfect to fill it out. Save the form, then either copy to disk or send as an e-mail attachment to Valerie Hughes.

If the dialog box does not open asking you to save the file you can right click (pc) or click and hold the mouse on the file (MAC) then choose:
