Application Forms

The forms on this page require Adobe Reader (version 5.0 or higher.) If you do not have Adobe Reader installed on your computer, it can be downloaded HERE for FREE. Follow the directions online to download the reader, then locate the installer on your computer and follow the directions for installing the application. Once the reader has been installed, you can view the form(s) and complete them online by placing your cursor on the first line and using the tab key to move from one field to the next. You can then print the completed form(s), sign where appropriate and deliver to: College of Education Office of Academic Services 206 Porter Bldg Ypsilanti MI, 48197 NOTE: These forms can be completed online, saved to your local disk, and printed out.

NOTE: If you plan to work on the student teacher form in the College of Education Bonisteel Computer Lab (205 Porter Bldg,) please be advised that the lab has a 10 PAGE MAXIMUM printing policy. Please print out one master copy of the form, then use a photocopier to make additional copies. Photocopiers are located in either the Educational Resources Center (100C Porter,) McKenney Union, or in Halle Library.

Teacher Preparation Program   • This application form is only for students who are currently earning their first bachelor’s degree & teacher certification simultaneously • This application form is NOT appropriate for post-baccalaureate, graduate, or master’s students • If you already have a bachelor’s degree & want to become a teacher, please contact 734.487.0275 for more information

Post-Baccalaureate Teacher Certification Program • This application form is for those students who already hold a bachelor's degree

Scholarships Available from Academic Services • Please type or print and return to 206 Porter by January 31, to apply for the folowing academic year • You may fill in this form online, but you will need to complete the whole form and print it out • You will not be able to save a copy • Each department in the college has its own scholarships and application
