As an EMU student who is being allowed to travel to various venues for a U.S. Travel Course you fully understand and appreciate the risks of injury which may result from your attendance during the trip and traveling to and from the various locations as part of the course. You voluntarily and knowingly recognize, accept and assume these risks. Furthermore, you, your heirs and any personal representatives agree to defend, indemnify and hold harmless, release and forever discharge EMU, its regents, officers, faculty, employees and agents, from and against any and all claims, liabilities, demands, personal injury or death which may result from participating in this course and traveling to and from our various locations.
Students under the age of eighteen require a parent/guardian signature agreeing to all terms and conditions. Copies of this form can be obtained from the U.S. Travel Coordinator in 101 Boone Hall.
Students who wish to cancel their registration must do so in writing to the U.S. Travel Coordinator located in 101 Boone Hall, Eastern Michigan University, Ypsilanti, MI 48197. The official date of cancellation will be the date the notice is received by our office; therefore you may wish to cancel by fax 734.487.6695 or e-mail email@example.com. There will be a $100 service fee for all cancellations after acceptance and registration in the program. Students withdrawing within 60 days of departure will be liable for 50 percent of total program cost. Students withdrawing within 30 days of departure, on the day of departure or during the program will be liable for the entire cost of the program with the exception of university tuition, which will be refunded according to current university policy.
