Birth, Adoption, or Legal Guardianship of a Child 

A Change of Status Life Event

Congratulations on your new arrival!  This Change of Status will allow you to make changes to and enroll in benefits mid-year. You have 30 calendar days from the date of the event to contact the Benefits Office to complete the necessary benefits change forms.  If you do not contact the Benefits Office within this time period, you must wait until the next available Open Enrollment period to make changes.  BC/BS PPO benefit deductions will be subject to taxes for any retro-active coverage. After that period, deduction costs will be pre-taxed. For your convenience, we have prepared a checklist of benefit changes permitted due to birth, adoption, or legal guardianship:

  • Add dependent child to Health Plan
  • Add dependent child to Dental Plan
  •    Consider changing beneficiaries of your
    • Life Insurance
    • Retirement plans (if participating in MPSERS –  www.michigan.gov/ors  or TIAA-CREF -www.tiaa-cref.org  )
    • Tax Deferred Annuities (if participating in TIAA-CREF or CHASE)
    • Travel Insurance (benefit applies to: AC/AP/PT/FAC)

All necessary forms can be obtained from and returned to the Benefits Office, 140 McKenny Hall, Ypsilanti, MI 48197, when completed. Please remember:

   Please include a copy of a statement from the hospital or your physician for verification of birth. In cases of adoption or legal guardianship, please include a copy of the court order

Marriage

A Change of Status Life Event

Congratulations on your recent marriage!  This  Change of Status  will allow you to make changes to and enroll in benefits mid-year. You have 30 days from the date of marriage to contact the Benefits Office to complete the necessary benefits change forms. A copy of your marriage certificate is required to verify the event.  If you do not contact the Benefits Office within this time period, you must wait until the   next available  Open Enrollment  period to make changes. If changing you last name, please submit a copy of new Social Security card reflecting the changed to the Staff or Academic Human Resources Office

  Divorce

If you lose coverage due to a divorce, you must contact the Benefits Office to complete the necessary benefits change forms within 30 calendar days from the date of the event. Provide a copy of the front page of the divorce decree. All necessary forms can be obtained from and returned when completed to the Benefits Office, 140 McKenny Hall, Ypsilanti, MI 48197. For your convenience, we have prepared a checklist of permitted benefit changes:

Death of a Family Member

A Change of Status Life Event

If the deceased was an active employee, a retired University employee, or a dependent of a University employee or retiree, please notify the Benefits Office at 734-487-3195 about your loss as quickly as possible with the following information so that we can process any applicable benefits

  •    Social Security number of the deceased
  •    Date of death
  •    Relationship of the deceased with the University
  •    Name, address, and phone number of the next of kin,if other than you

Dropping of Coverage for Your Spouse

A Change of Status Life Event

Voluntary

Medical and dental insurance coverage for your spouse may be dropped at any time without proof of insurance elsewhere. However, a written notice to the Benefits Office is required in order to voluntarily cancel coverage. COBRA notification will be sent.

Divorce

If you and your spouse get divorced, you must remove that person from your benefits. The deletion must occur within 30 days of the divorce in order for COBRA continuation information to be sent to your former spouse. Please be sure to provide us with a copy of the front page of your divorce decree and your former spouse’s new address for COBRA notification. If you wish to change your last name, a copy of your new Social Security Card is needed. For your convenience, we have prepared a checklist of benefit changes permitted due to a divorce:

Spouse's Loss or Change of Employment

A Change of Status Life Event

This  Change of Status  refers to any event that results in your loss or gain of eligibility for medical and dental coverage and will allow you to make changes or enroll in benefits mid-year. If you lose coverage due to your spouse’s termination or change of employment, you have 30 days from the date of the event to contact the Benefits Office to complete the necessary benefits change forms.  If you do not contact the Benefits   Office within this time period, you must wait until the next available  Open Enrollment   period to make changes. For your convenience, we have prepared a checklist of benefit changes permitted due to this  Change of Status  life event:

  •  Enroll or change coverage for yourself, spouse and/or dependent(s) in Health Plan
  •  Enroll or change coverage for yourself, spouse and/or dependent(s) in Dental Plan
