Health Care Plan Waiver Of Coverage

Eastern Michigan University (EMU) employees whose spouses have health care coverage through another employer or who have health care coverage elsewhere may, upon documented proof of coverage, decline Eastern Michigan University coverage and receive a monetary reward (pro-rated amount if hired after January 1st).  This waiver amount will be added to payroll checks and is fully taxable. Please see chart below for monetary rewards.

EMU employees must initially present a letter from their spouses' employer or other insurance carrier verifying their medical coverage.  Letters must include name of carrier, employer and type of coverage (single, two person/family).  Coverage through COBRA will not be accepted.  In addition, employees must sign a Health Care Plan Waiver of Coverage form (available on the HR Forms page, or you can obtain a form from the Benefits Office. This form will remain in effect until the employee makes a change.  You will no longer have to submit an annual notification of other coverage after this enrollment period.

Should the employee lose coverage through his/her spouse as a result of a lay-off, termination, death, divorce or cancellation of the plan, the employee will be allowed to elect coverage through Eastern Michigan University's plan upon notification within 30 calendar days of the loss of coverage.  The appropriate documentation and waiver form must be turned in to the Benefits Office. Employees may also elect to return to Eastern Michigan University's health plan during the annual open enrollment period.

FA Employees ...............................................................$1,000 annually
