10-1304 Form(.doc)
This form is needed for F-1/J-1 students who have found on-campus employment and need to apply for a Social Security Card. The hiring department needs to print this form on their departmental letterhead.

Online Course Stipulation

If an international student is taking more than one online course for a term/semester, his/her Academic Advisor and the Professor of the specified online course has to sign this form and the student has to submit it to OIS .
