How to open a bank account? A: In opening a bank account, you need to bring your passport and ID such as Michigan ID or MI driver’s license, social security number, and other immigration-related documents to the bank, fill in the application form (for checking and/or saving accounts)

is Form W-8Ben? A: Form W-8Ben is a form that non-US citizen need to fill out for tax exemption on interest earned on your account. You should ensure the bank officer provide you this form when opening a new bank account or you can download it at IRS website.

If I get sick at night or
weekends and I cannot wait till
Snow Health Center open, what
should I do? A:
You should go to urgent care
section at one of the providing hospitals.
If you go to the emergency room for a mild illness, the insurance
company will not fully reimburse the cost you have paid.

How do I check out materials? A:
Take them to the reception desk on the 1st level by the
Halle’s entrance. Remember

New students can create their own EMU student accounts for email and

together? A:
There are nearly 30 Collaborative Study Rooms on the 2nd and
3rd Levels. Rooms come with wipe boards and are on the first

word processing? A:
Bring your eagle card I.D. to the Computing Commons ( Ground Level ).
Windows and Mac workstations are available with staff ready to help you.

Go to the Multimedia Self-Production Area ( 1st Level ).
Staff at the Multimedia Services Desk ( 106 Halle ) will be happy to

Where can I go to get help finding
periodical articles? A:
Ask a librarian at the Information Desk ( 1st Level ) for
assistance. Periodicals (magazines and journals) are arranged in
alphabetical order on the 2nd Level. Librarians can also show
you how to access some of the 90+ online databases and indexes in
Voyager. Some include the full text of articles. Q:

What is I-9 Form? A: I-9 Form is the form that includes questions about EMU’s rules and important information you have to know before you work. You can get this form at OFSA. Q:
How can I find a job on campus? A:
There are many ways to find a job on campus.
First, you should look at a notice on job board at the
place that you want to work. Second,
you may ask people who work on campus how you can apply that job.
Finally, you can find out job on Echo
newspaper that is EMU’s newspaper.

Q:
How can I apply the curricular
practical training? A: You should follow this procedure 1.
Obtain an attestation by the academic department on a COOP’s
student agreement form or the OFSA’s CPT contract. 2.
Request your employer to complete and sign an agreement with the
COOP office or OFSA’s CPT contract. 3.
Submit a completed CPT Work/Learning Agreement Form or the
OFSA’s CPT contract, Form I-538, and your I-20 to the DSO for getting
approval. After that, you
can start your work.

What should I do when I want to apply optional practical
training? A:
You should follow this procedure: 1.
Obtain a recommendation letter or memo from your academic advisor
or head of department. 2.
Complete Forms I-538 (two), I-765, I-765 Signature Card with two
recent photos according to INS specifications.
3.
Prepare check or money order for required fee. 4.
Meet with the DSO along with all documents and I-20 for
recommendation. 5.
Process for INS approval.
