Do you want your home address and/or home telephone number listed in the green pages of the printed campus directory?

Let us know what personal information, if any, you want to have listed in the 2006-2007 printed telephone directory. Please fill out this form completely and send it to University Communications at 18 Welch Hall by Sept. 15, 2006.

Your home address and telephone number on file with human resources can be verified and updated through your my.emich account. If the information needs to be updated, you must initiate that change through your my.emich account in order for the change to appear in the directory. Do not write home address or telephone numbers on this form as we do not have the ability to make such changes. The purpose of this form is only to let us know what information you want included in the printed telephone directory. Your E-number can be found on your faculty/staff I.D. card.
