1. The policy covers Illness and Injury.

2. Preventive medicine, general physicals and immunizations are NOT covered. See brochure for
details of exclusions.

3. Health benefits coverage up to $50,000 per Injury or Sickness.

4. Additional $50,000 coverage option may be purchased.

5. Patients pay a $10 co-pay per visit for covered services at Snow Health Center, including office visits, lab tests, and x-rays.

6. Local area hospital and physician participation.

7. Provides coverage worldwide.

8. Medical Evacuation and Repatriation provided by ASSIST AMERICA GLOBAL EMERGENCY
MEDICAL ASSISTANCE.

9. The patient will incur a $100 deductible for each injury or sickness when services are obtained outside of SNOW HEALTH CENTER.

10. Prescriptions filled at Snow Health Center will have a $10 co-pay for generic drugs and a $20 co-pay for name brand drugs prescribed by a Snow Health Center Physician.
