The policy can be purchased for an entire academic year for $943.00
This would be from September 1, 2009 - August 31, 2010

The policy can also be purchased by the semester for $315 per semester.
- Fall: September 01, 2009 - December 31, 2010
- Winter: January 1, 2010 - May 1, 2010
- Spring/Summer: May 01, 2010 to august 31, 2010

Policy Quick Facts

1. The policy covers Sickness and Injury.

2. The policy covers one (1) annual wellness exam per Policy Year when performed at the Snow Health Center. Further preventive medicine, general physicals and immunizations are NOT covered.

3. Health benefits coverage up to $50,000 per Injury or Sickness with an option to purchase an additional $50,000 of coverage.

4. Patients pay a $10 co-pay per visit for covered services at Snow Health Center, including office visits, lab tests, and x-rays.

5. Preferred Physician, Hospitals and other health care provider participation.

6. Provides coverage worldwide.

7. Medical Evacuation and Repatriation provided by SCHOLASTIC EMERGENCY SERVICES, INC.
GLOBAL EMERGENCY MEDICAL ASSISTANCE.

8. The patient will incur a $100 deductible for each injury or sickness when services are obtained outside of SNOW HEALTH CENTER.

9. Prescriptions filled at Snow Health Center will have a $10 copay for generic drugs and a $20 copay for name brand drugs prescribed by a Snow Health Center Physician.

10. This flyer provides a brief summary of benefits provided under Plan 2009-1437-1. Refer to the attached brochure for coverage provided when obtaining services outside Snow Health Center.
