Forms
- Please download the editable forms in order to access the digital signature lines.
- All forms are requests, not automatic approvals.
- Please allow 48 business hours processing time to receive confirmation or feedback.
- Groups must wait for confirmation from our office in order to proceed with marketing and planning.

EMU Student Organizations, in good standing with Campus Life, may also need these additional documents:
- ES Student Organization Profile Update [Editable PDF] - To keep ES updated with current advisor, president, and members who can book spaces for your org.
- 2023-2024 Multi-Date Meeting Request Form [Editable PDF] - To request multiple dates for a standing (repeating) meeting . The time frame can include up through the entire academic year, both fall and winter.
