Absence Notification

When a student must miss class for an extended medical, mental health or personal reason, the Dean of Students Office can notify faculty. Students must submit an absence notification form, a completed and signed confidentiality waiver and proper documentation for the absence. Please print or type legibly.

Absence notification requests can be submitted for the following reasons:

  • Physical health issues
  • Mental health issues
  • Personal crises
  • Death of family member or significant other

Absence notification will be sent to professor within 24-48 hours.

 

Absence notification requests should not be submitted for the following reasons:

  • Travel issues
  • Weather-related delays
  • Jobs/Internships
  • Court appearances
  • Pet related issues

In these cases, students should speak directly with their faculty.

Available Forms

The absence notification does not mean the student is excused from course material, assignments or exams. It is ultimately up to each professor to decide what, if any, accommodation can be provided in light of this absence. It is the student’s responsibility to follow up with the faculty member to inquire about such accommodations.
