Step 1: Intake Form
Complete the intake form and submit supportive medical documentation.
Step 2: Documentation
If you have difficulty uploading documentation, please email documents to [email protected].
Step 3: Intake Meeting
Once all documentation has been submitted, please know a member of our staff will reach out to you and set up an intake meeting to look over the accommodations we can provide.
