Sexual Misconduct and Sex-based Discrimination Reporting Form

Please fill out the form fields below and, to the best of your ability, thoroughly describe the behavior you observed and any other information that you have about the incident.

When reporting an incident via this form, please note the following:

  • This reporting form is for non-emergency incidents only. If this is an emergency situation, dial 911 or contact the EMU Police at 734.487.1222.
  • The Sexual Misconduct Prevention and Response Office (formerly Title IX Office) will make reasonable efforts to respect and protect your privacy. However, responding to violence and harassment may require collaboration across different areas of expertise on campus. If the reporting person(s) submit their contact information in their report online or directly to the Title IX Coordinator, they may be informed of the need of necessary additional information to appropriately address the report.
  • EMU regards reports made through this form seriously. Following the submission of the form, the reporting party can expect an outreach of support from the Sexual Misconduct Prevention and Response Office. If you have questions, please email the Title IX Coordinator, Matthew Gregory, mgregory@emich.edu.

NOTE: The Sexual Misconduct and Sex-based Discrimination Reporting Form was developed to receive incident reports as a part of the intake process. Submitting this form will not automatically initiate a University investigation of the reported conduct, rather, this form serves as a method to notify the University of possible Title IX violation(s) and enables the Sexual Misconduct Prevention and Response Office to provide information about resources, support, and potential next steps. The Sexual Misconduct Prevention and Response Office will notify the persons named in the report prior to the start of a University investigation.


Report an Incident

There are two (2) options for reporting an incident online:

Option #2: Reporting with Identifiable Information

If you provide identifiable information, the Sexual Misconduct Prevention and Response Office (formerly Title IX Office) will contact the reporter to acknowledge receipt of the report. Additionally, the Title IX Coordinator will reach out to the named person who received harm to provide information about resources, support, and potential next steps. The Sexual Misconduct Prevention and Response Office also offers equitable support services for all individuals named in a Title IX report.

Option #1: Reporting Anonymously

The EMU Sexual Misconduct Prevention and Response Office (formerly Title IX Office) accepts anonymous reports; however, please be aware, if the reporter chooses to report anonymously and does not provide thier name or other contact information, the Sexual Misconduct Prevention and Response Office may be limited in its ability to respond to the reported information.
