  • Welcome to the Digital Divas Documents Center. All forms required for an organization's attendance at Digital Divas can be found on this page. To save time for teachers, administrators, and parents, portions of the forms have been pre-filled for our admitted organizations under their respective drop-down tabs. Blank forms (not pre-filled) and supplemental forms, like our transportation aid application, are found at the bottom of the page.

 

Document upload deadline is Friday, October 11, 2019 by 5:00 p.m.


 

Pre-filled Forms

To save time for teachers and organization leaders, admitted groups may download partially pre-filled registration documents for your organization. Pre-filled sections include areas that ask for the organization name, organization ID number, and the organization's main contact. 

  1. Select your organization from the options below. 
  2. Download the pre-filled organization specific roster, student waiver, and chaperone waiver.
  3. Have all parties complete waivers while main contact builds the roster.
  4. Upload all waivers and the roster to your organization's password-protected document upload area. 
    • Note: Passwords are sent in your organization's admissions email to the main contact listed on the Digital Divas Application. For security purposes, the Digital Divas team will not release upload passwords to any other person. If working as part of a district, please contact the person that applied on your school's behalf.
