Special announcement: The Spring 2020 Digital Divas conference will be virtual and no registration is required. No information is required to be submitted like we normally require for in-person conferences.

Welcome to the Digital Divas Documents Center. All forms required for an organization's attendance at Digital Divas can be found on this page. Teachers, administrators, and students, may pre-fill forms and print typed information before taking the form home for a parent/guardian signature. Other documents and supplemental forms, like our transportation aid application, are found at the bottom of the web page.

Pre-filled Forms

To save time for teachers and organization leaders, admitted groups may pre-fill registration documents for your organization.

  1. Click on the appropriate forms below to begin filing out fields online.
  2. Print the completed form and collect the required signatures. Teachers/ the lead chaperone should collect all forms before uploading.
  3. Scan the completed forms. Forms must be uploaded in one batch and each participant's files should be front-to-back. Do not separate the two forms for participants.
  4. Teachers: Upload all waivers to your organization's password-protected document upload area.
    • Note: Passwords are sent in your organization's admissions email to the main contact listed on the Digital Divas Application. For security purposes, the Digital Divas team will not release upload passwords to any other person. If working as part of a district, please contact the person that applied on your school's behalf.

Document Upload Center

Teachers, school administrators, or other lead chaperones may upload their group's waivers by locating their organization from the menu below. To protect participant information, each organization will have their own password protected upload button. The upload center will be fully functional once admissions notifications are sent the first week of February 2020.
